These podcasts, excerpted from an August 2022 webinar, discuss strategies for providing coordinated care for individuals with I/DD, including person-centered medical assessment considerations for individuals with I/DD, risk stratification strategies to drive interventions, and family or caregiver support, education, and outreach. Speakers describe the current gaps in care for individuals with I/DD and share strategies for delivering person-centered care for individuals with I/DD through use of interdisciplinary teams (IDTs) and innovative care coordination models.
