In 1966, two Brazilian men were found dead on Vintém Hill under bizarre circumstances that continue to perplex investigators and conspiracy theorists alike. Lying side by side, their bodies were discovered wearing matching lead masks—shields with no eyeholes—alongside cryptic notes. 🚨 Are you using all three types of downtraining in your practice? 🚨 We often talk about "downtraining" without fully defining what it is—or recognizing that there are actually three distinct types . If you’re not considering all three, you’re missing key ways to help your patients. We’re bringing back one of our most popular episodes! While I’m out on maternity leave, we’re replaying some of the most impactful conversations we’ve had on the podcast over the years. In this episode, we break down: ✅ Pelvic Floor – Direct – Hands-on techniques, biofeedback, and other direct interventions. ✅ Pelvic Floor – Indirect – Treating surrounding structures like the hips, glutes, and breath mechanics. ✅ Central Nervous System – Addressing the brain-body connection, pain science, and autonomic regulation. Each type of downtraining requires different techniques —and knowing which one to prioritize can get your patients better, faster . If you’ve ever felt like “downtraining” is a vague, overused term, this episode will give you clear, actionable strategies to make it more effective for your patients! Downtraining Masterclass If you'd like a deep dive into downtraining, make sure you check out the Downtraining Masterclass! (www.pelvicptrising.com/downtraining). Not everyone will understand your cash-based practice. That’s okay. In this episode, we’re talking about how to handle the naysayers, critics, and confused looks that come with running a cash-based pelvic health practice. Whether it’s a skeptical practitioner, a price-sensitive patient, or your uncle who still doesn’t really get what you do—we want you feeling confident in the model you’ve chosen. We break it down by type: ✅ Other practitioners – How to market and explain what you do in a way that’s funny, honest, and real , without being defensive or weird about money. ✅ Patients – Giving them everything they need to make an informed choice and respecting that you’re not the right fit for everyone. That’s a good thing. ✅ Friends & family – Real talk: they have no idea what we do. Love them. But don’t let their opinions shake your confidence. You don’t need everyone to get it. You just need to own your decision and know your value. 🎧 Business Accelerator Program - Next Cohort in July! Is biofeedback really worth your time? Biofeedback has been a staple in pelvic therapy for years, but is it actually helping patients—or is it just another outdated tool that doesn’t deliver results? This is a replay of one of our most thought-provoking episodes! While I’m out on maternity leave, we’re bringing back some of the best conversations we’ve had over 500+ episodes. In this episode, Nicole shares her biggest frustrations with biofeedback , including: 🚫 It focuses entirely on the pelvic floor and ignores the full-body system. 🚫 It’s all about “strengthening” —but we know the pelvic floor is more than that. 🚫 It rarely works properly (you’ve probably struggled with faulty readings too!). 🚫 It’s internally inconsistent —the same person can get wildly different results. 🚫 It isn’t functional —training isolated contractions doesn’t translate into real-life movement. We also break down the best arguments for and against biofeedback , looking at both sides of the debate. So what do you think—does biofeedback have a place in pelvic health? Or is it time to move on? Let’s dive in! About Us Nicole and Jesse Cozean founded Pelvic PT Rising to provide clinical and business resources to physical therapists to change the way we treat pelvic health. Thinking about going cash-based—or already there but questioning it? Let’s break it all down. In this episode, we’re talking through the pros and cons of being in a cash-based practice . We break it down into three major categories: ✅ Clinical – The biggest win. You get to treat patients how you actually want—no insurance companies dictating your plan of care or cutting off treatment early. You’re in control, not a profit-focused third party. ✅ Financial – Cash-based practices have way higher profit margins . You can charge what you're worth (not what insurance allows), and you don’t need a massive patient volume just to stay afloat. ✅ Lifestyle – Fewer patients. More time. More flexibility. When your margins are higher and your schedule is your own, you get to live more life . BUT—it comes with a catch. You’ve got to get patients in the door . That means marketing, messaging, and selling your value. That’s the deal. 🎧 Tune in to hear what makes cash-based worth it (and where you need to step up to make it work). Hiring a great admin is just the first step—how you onboard them is what really matters. The first few weeks are critical in making sure your new front desk hire becomes a high-performing, confident part of your team. If you don’t set them up for success, you’ll find yourself micromanaging, frustrated, or even worse—back at square one looking for someone new. In this final part of our Front Desk Series , we break down how to train your new admin in a way that’s efficient, clear, and sets them up for long-term success . Here’s how we do it: 📌 Step 1: Get them comfortable with the logistics of the job and their day-to-day responsibilities. 📌 Step 2: Teach them how to confidently help existing patients with scheduling, billing, and superbills. 📌 Step 3: Train them on handling new patient calls using our primary and secondary phone scripts and call logs. 📌 Step 4: Introduce systems and processes—drop-off lists, supply orders, thank-you notes, marketing letters, and more. We also share pro tips for onboarding , including: ✅ Give them the big picture vision of the practice. ✅ Record trainings to make future hires easier. ✅ Don’t overreact to early performance (good or bad). ✅ Do the training yourself —this is not something to outsource. Your admin should be fully up to speed within two months and operating at a high level by three months . We also discuss key success metrics —phone conversion rates, patient booking rates, and the practice’s overall fill rate. If you want a front desk team that truly supports your practice, this is the episode you can’t miss! The pelvic floor isn’t just another muscle—so why do we treat it like one? The pelvic floor is different from every other muscle or area in the body, and if we don’t acknowledge that, we won’t get the best results for our patients. This is a replay of one of our most thought-provoking episodes! While I’m out on maternity leave, we’re bringing back some of the best conversations we’ve had over 500+ episodes. In this episode, we break down five key ways the pelvic floor is unique : ✅ Autonomic innervation – The pelvic floor has both voluntary and involuntary control, making treatment more complex. ✅ Unique anatomy & function – It doesn’t have a typical origin/insertion like other muscles. ✅ Interconnected with organs – Unlike most muscles, the pelvic floor is woven into multiple organ systems. ✅ Emotional & psychosocial components – Trauma, stigma, and cultural attitudes impact pelvic health treatment. ✅ You can’t rest it – The pelvic floor is always in use, which makes healing more challenging. When we understand these differences, we can treat the pelvic floor more effectively —instead of relying on outdated approaches that don’t take its complexity into account. Tune in to hear why there’s no such thing as a simple pelvic floor patient! Hiring a front desk admin? You don’t have time to waste. One of the biggest challenges in hiring an admin is sorting through a flood of applications and avoiding interviews with people who aren’t actually qualified or interested (just looking for a paycheck!). In this episode (Part 2 of our Front Desk Series), we walk through the exact 5-step hiring process we use at PelvicSanity to find the best admin—without spending hours on bad interviews. Here’s how it works: 📌 Step 1: Post the job (Indeed, social media, email list) and require a cover letter to filter out low-effort applicants. 📌 Step 2: Send a Google Form questionnaire to narrow the pool based on what matters to them in a job. 📌 Step 3: Do an informal Zoom interview to screen for basic fit before an in-person meeting. 📌 Step 4: Bring in 3-4 finalists for an in-person interview to sell the position and ensure a good match. 📌 Step 5: Do a working interview to test their skills, how they learn, and how they interact with patients. At the end of the day, we pick the best candidate and trust in our ability to train them (and replace them if needed!). If you’ve struggled with hiring a great admin—or wasted time on bad interviews—this episode will give you a streamlined, proven hiring process! 1 The Pelvic Floor is a Trampoline, Not a Biceps - A Better Analogy 22:52

🚨 Are we still using the biceps analogy for the pelvic floor? It’s time for an upgrade. When explaining pelvic floor strength , many of us were taught to compare it to a bicep—contracting, strengthening, and controlling movement in a straightforward way. But this analogy doesn’t quite capture the full picture. This is one of our favorite episodes from the past 500+ we’ve recorded! While I’m out on maternity leave, we’re replaying some of the most impactful conversations we’ve had on the podcast over the years. Instead, I teach the trampoline analogy in my Essential Pelvic Strengthening: Not Your Mama’s Kegels course. Here’s why it works better: ✅ Explains resting tone – The pelvic floor isn’t just about contraction; it has a natural resting position. ✅ Shows how surrounding structures matter – Just like a trampoline depends on its frame, the pelvic floor is influenced by the core, glutes, and breath. ✅ Highlights involuntary movement – The pelvic floor reacts automatically to forces like pressure and movement, just like a trampoline responding to weight. This analogy helps patients better understand their bodies, making it easier to explain treatment and exercises in a way that actually clicks. Tune in to hear why I think the trampoline analogy should replace the old-school biceps analogy —for both clinicians and patients! Essential Pelvic Strengthening If you're looking for more information on how to strengthen the pelvic floor without relying on Kegel exercises and how to make this more functional for patients, make sure to check out the Not Your Mama's Kegels course! www.pelvicptrising.com/kegels About Us Nicole and Jesse Cozean founded Pelvic PT Rising to provide clinical and business resources to physical therapists to change the way we treat pelvic health. Are you holding your business back by NOT hiring an admin? We were way too late in hiring a front desk team at PelvicSanity, and it’s one of the biggest mistakes we see practice owners make. If you’re unsure whether you need an admin, ask yourself: 👉 What aren’t you able to do for your business because you don’t have time? If you have answers to that question, it’s time to start looking for help. In this episode, we break down: ✅ Why an admin is necessary if you want to grow beyond yourself ✅ How a front desk team actually adds value (not just “costs” money) ✅ Why hiring an in-person admin is the gold standard over a virtual one ✅ The mistake of having clinicians do admin work (it’s a waste of their skills!) ✅ Why you’ll need more admin hours than you think Whether you’re a solopreneur or planning to expand, bringing on admin help is one of the smartest investments you can make. Still starting your evals with strength testing? It’s time to rethink that. Most pelvic health pros are trained to assess isolated pelvic floor strength first—and then jump straight into isolated strengthening. But we believe that’s backwards . In this episode, we’re talking about why you should start with function first —because it leads to better patient buy-in, faster results, and helps your patients actually connect their symptoms to real-life movement and function . We also dive into: ✅ Why pelvic floor therapists often overlook orthopedic factors ✅ Why ortho therapists often miss the specialized pelvic floor knowledge ✅ How combining both orthopedic and pelvic assessments gives you the full picture ✅ The importance of standing and dynamic assessments to see how the pelvic floor works in the system ✅ Our mantra: Assess, Don’t Guess! Here’s why: 🚫 Google prioritizes local SEO – Your blog won’t really help you rank for local searches. 🚫 AI-generated content dominates search results – People aren’t even clicking blogs anymore. 🚫 The internet is flooded with AI-written blogs – You can’t compete with mass-produced content. 🚫 Blog readers aren’t local – They’re searching for information , not a pelvic health provider. So what should you focus on instead ? ✅ A website that converts visitors into patients ✅ Optimizing your Google Business Profile ✅ Getting patient reviews ✅ Building a strong FAQ page that sells your services ✅ Listing your practice in local directories and social platforms ✅ Real, local marketing—networking, events, and partnerships Don’t waste your time (or money) writing blog posts no one will read! Tune in to learn what actually works to grow your practice. Isolated pelvic floor strength isn’t the key to pelvic health. In Part 2 of the Great Kegel Debate , we dive deeper into why we come down firmly on the anti-Kegel side —and why we believe pelvic health is about so much more than isolated strength. The single biggest reason? Strength has no reliable correlation with symptoms. 👉 We’ve all seen patients with objectively weak pelvic floors who have no symptoms at all . 👉 And others with strong pelvic floors who are dealing with significant dysfunction . If isolated strength doesn’t predict symptoms, why would it be the primary focus of treatment? We also share why we aren’t blindly rejecting Kegels—we understand both their pros and cons. In fact, we hold these two ideas at the same time: 💡 Someone could need pelvic floor strengthening… AND it’s not the most important thing to do right now. 💡 You can have weak pelvic floor muscles… AND be totally symptom-free. 💡 A muscle can have volitional control… AND still work primarily involuntarily. 💡 Something can be proven in the literature... AND there can still be a better way to do it. 💡 We can value what research says.... AND realize research always lags behind (often by decades) clinical practice. 💡 There are times when Kegel exercises are incredibly important and the most effective tool for a patient... AND recognize these times are rare and the exception that proves the rule. We also explain how testing voluntary muscle strength at the initial evaluation can lead us down the wrong treatment path —and what to focus on instead. This is a must-listen for anyone rethinking how we approach pelvic floor strength in our evaluations and treatment! Are packages really the best way to sell pelvic health services? At first glance, packages sound like a great idea. They give patients commitment, help cash flow, and make sales "easier," right? Not so fast. In this episode, we break down why we’re opposed to selling packages in pelvic health and the major issues that come with them: ❌ Unrealistic patient expectations – Patients assume they’ll be “done” when the package ends (even if they’re not). ❌ You’re putting limits on care – We hated when insurance dictated treatment plans, so why are we doing it to ourselves? ❌ Cash flow challenges – You get paid upfront, but then have to deliver care without ongoing revenue. ❌ Hiring headaches – Most great clinicians don’t want to be salespeople, and training them to sell packages is hard . At the end, we share what to do if you’re committed to selling packages and how to structure them for the best results. But after working with over 650+ practices , we’ve seen very few do it well. If you’ve been debating packages, this is the episode you need to hear first! The Kegel debate is heating up… but what are we really disagreeing about? Over the past few years, the conversation around Kegels has shifted big time . We’ve seen an explosion of “no-Kegel” content, frustration from those pushing back, and even research articles challenging each other in the literature. In this episode, we break down the three major camps in the Kegel conversation: 🔥 Pro-Kegel – Kegels are the most researched pelvic health tool and should be used for almost everyone. 🔥 Anti-Kegel – There are better , more functional ways to strengthen the pelvic floor. Kegels shouldn’t be a blanket prescription. 🔥 Anti-Anti-Kegel – Stop telling people not to do Kegels! They have limitations but are a valid part of treatment. But here’s the thing… we actually agree on way more than we disagree on! We all know: ✅ Kegels can be useful—but they’re not for everyone. ✅ Pelvic floor therapy is so much more than Kegels. ✅ Kegels should never be blindly prescribed without assessment. ✅ Movement and functional training are critical for success. So why all the controversy? A lot of it comes down to how we talk about this on social media —and whether we should be the "Instagram police" calling out others’ content. Let’s get into the real conversation and find the common ground in the Great Kegel Debate !
